Medicaid Service Coordination:

Many of the services needed by individuals with disabilities are funded through Medicaid. Working with families and individuals Medicaid Service Coordinators develop and implement individualized plans for success based on their desires, interests and needs of the people we serve.

Our services are tailored to each individual’s unique needs and desires. The Medicaid Service Coordinator meets with individuals in their homes and communities on an on-going basis. The Individual service plans allow those in our program to thrive, grow and live more independent lives by focusing on consumer satisfaction and planning accordingly to the individual's needs.

Our Services Include:

• Developing, implementing and monitoring an Individualized Service Plan (ISP) that identifies the individual’s goals and needs.
• Setting goals to develop greater independence, inclusion and productivity.
• Monitoring the individual’s health and safety needs.
• Providing a variety of options, opportunities and resources for supports and services and helping people get those that are needed or desired.
